Guidelines for the Ohio
Dental Assistants Certification Examination.
Qualifications for
applicants for the Ohio Dental Assistants Certification Examination
The Ohio Certification Examination will be
available to any individual who meets one of the following:
- Has a minimum of six (6) months
continuous full time work experience
- Is enrolled in the second year of a two
(2) year Dental Assisting program
- is enrolled in a Post High School Dental
Assisting program.
IN ORDER TO BE EXAMINED
THE INDIVIDUAL MUST MEET THE FOLLOWING REQUIREMENTS:
- Provide proof of meeting the
professional ethics requirement.
- File a correct and complete application
prior to the deadline for the application
- Pay the correct fee by check or money
order.
- Proof of current CPR certificate.
- Submit two passport (2"x2")
pictures - no others will be accepted.
IN ORDER TO ESTABLISH
ETHICAL CONDUCT THE APPLICANT MUST MEET ONE OF THE FOLLOWING.
- Hold membership in one of the following:
ODAA, ADAA, ADHA, NDHA.
- Submit THREE NOTARIZED LETTERS from
current members of following organizations: ODAA, ADAA, ODA, ADA.
- Students enrolled in a Dental Assisting
program - one notarized letter from your instructor.
IN ORDER TO BE CERTIFIED THE INDIVIDUAL
MUST MEET THE PRECEDING LISTED REQUIREMENTS FOR THE EXAMINATION AND MUST
PASS THE CLINICAL, WRITTEN, AND RADIOLOGY PORTIONS OF THE EXAMINATION.
FEE REQUIREMENT
The fee for the examination will be $50.00
and must accompany the application.
Applicants who fail a portion of the
examination will be required to retake only the portion failed. The fee to
retake the examination will be $40.00
Applicants who apply for the examination
and who are not able to take the examination at the scheduled time may
request to be rescheduled at a later date, and will be required to pay a
$5.00 reprocessing fee.
Examination fees are not refundable.
Failure to take the examination or failure to meet all requirements after
submitting the examination application and fee are not cause for refund.
All fees must be paid by either check or
money order, and payable to the Commission on Ohio Dental Assistants
Certification, Inc.
N.S.F. checks will incur an additional
charge of $20.00
EXAMINATION APPLICATION
An examination application and exam dates
may be obtained from the:
Commission on Ohio Dental Assistants
Certification, Inc.

Applications must be received 45 days prior
to the exam date applied for, and must be completed in full. Applications
received after the deadline will be held until the next scheduled
examination date. Each Exam date is limited to 50 applicants on a first
come basis.
Persons with disabilities needing
assistance are asked to notify the Commission in writing at the time of
application.
CERTIFICATION EXAMINATION
CONTENT
The examination is composed of an objective
and clinical performance tests. The administration and grading of the
examination is done by the examiners appointed by the Commission on Ohio
Dental Assistants Certification, Inc. The objective test will be a written
multiple choice examination consisting of approximately 350 questions. The
written and/or clinical portions of the examination may consist of any or
all of the following areas.

All equipment and supplies will be
furnished.
UNIFORM REQUIREMENTS FOR
CLINICAL EXAM
- PROFESSIONAL UNIFORM (OSHA Standards)
- Clean white shoes
- Appropriate undergarments
- No nail polish
- Hair off of collar and away from face
- Name tags should NOT be worn for
examination
Jewelry
- One pair of small earrings
- Watch
- One Ring
Failure to meet all uniform requirements
will result in ten (10) points reduction on applicant's clinical score.
GENERAL RULES FOR THE
EXAMINATION
- No questions concerning the content of
the written test may be asked during examination sessions. The
candidates should listen carefully to all instructions and follow
directions carefully.
- No reference materials or notes may be
brought to examination sessions.
- No visitors or unauthorized individuals
will be permitted during testing sessions.
- Candidates taking the clinical
portion of the examination MUST BE IN UNIFORM.
- The examination will require
approximately six hours.
- Applicants arriving late for the
examination will be admitted but will not be allowed extra time beyond
the time scheduled for completion of the examination.
- Applicants for the examination will
receive information as to the exact date, time, and address for the
examination approximately ten days prior to the examination.
- All address changes should be promptly
reported to the Commission on Ohio Dental Assistants Certification,
Inc.
- Any request for information about either
the examination or certification should include the applicant's name
at the time the examination was taken and their Social Security number
- No examination results will be released
if all requirements are not met.
- Examination results will be released
only in writing and not be telephone, and only to the applicant unless
otherwise requested by the applicant.
- The Commission on Dental Assistants
Certification, Inc. will not discriminate in application, examination
or certification activities on the basis of age, sex, race, color,
religion, national origin, handicap, sexual preference, or marital
status. The birth year of all applicants is requested as part of the
identification data.
- Applicants who believe their scores from
the Certification Examination were not properly recorded or who feel
that test conditions adversely affected their performance on the
examination may forward any complaints in writing to the Chairperson
of the Board of Dental Assistants Certification, Inc. All decisions
made by the Board of Directors will be final.
- Candidates are responsible for
protecting the integrity of their answers. If cheating is detected
during the examinations, or evidence of cheating is disclosed at the
time or when the answer sheets are being scored, those involved,
whether they be copiers or those copied from, will be failed. This
warning notice will be in effect and applicable to all parts of the
examination.
- Applicants failing the examination three
times will be required to complete additional education before
retaking the examination the fourth time. Such applicants should
contact the Commission on Ohio Dental Assistants Certification, Inc.
for appropriate information.
- No application will be processed until
the examination fee is recevied by the Commission on Ohio Dental
Assistants Certification, Inc.
- Examinations shall be held on dates and
in locations specified by the Commission on Ohio Dental Assistants
Certification, Inc.
- The application deadlines will be
strictly enforced. Applicants should allow adequate mail delivery time
to insure that the application reaches the Commission prior to the
deadline. Applications received after the deadline will be handled by
scheduling the applicant for the first testing date available
following the date requested.
