Cost and Specifics of Services

DOT required SAP Evaluation Services  $400. Includes up to 3 hours face-to-face evaluation and follow-up time, letters to employer, collateral contacts and screening tools. Referrals made to appropriate educational or rehabilitation interventions.

COATS required SAP Evaluation Services $300. Includes up to 2 hours face-to-face evaluation and follow-up time, letters to employer, collateral contacts and screening tools. Referrals made to appropriate educational or rehabilitation interventions.

Receive 10% discount for either of these services with prepayment by internet request below only. Good for 10 days. No insurance or personal checks for these services. Payment in full at time of service with cash or VISA/MASTERCARD.
