HealthPoint offers affordable medical care to patients not yet
eligible for Medicare, based on income and family size. The charge is only
$15-$25 for office visits if your annual income is:
Under $17,179 for 1-person household
Under $23,219 for 2-person household
Under $29,259 for 3-person household
Under $35,299 for 4-person household
If you are currently covered by Medicare you will not be charged the $100 yearly Medicare deductible. The 20% co-pay for office visits is not charged if your annual income is:
Under $17,000 for 1-person household
Under $23,879 for 2-person household
Under $30,039 for 3-person household
Under $36,199 for 4-person household

This is a statewide non-profit organization that coordinates access to free health
care for citizens who do not qualify for government programs and cannot
afford health insurance. Once a person is approved, he or she remains eligible
for one year to visit volunteer doctors, dentists, hospitals, home health
agencies and hospices, and obtain free prescriptions donated by pharmaceutical
companies and filled by retail and hospital pharmacies. An individual can
reapply for another year of eligibility at the end of the first year.
Eligibility
Must be a Kentucky resident and:
Uninsured (without private insurance or public assistance through Medicaid or Medicare)
Income below 100 percent of the federal poverty guidelines (see below)
Resource limit of $2,000

Seeing A Physician Or Getting A Referral To A Specialist
Once a person has been determined eligible for the program, he or she can call
the Kentucky Physicians Care Hotline at 1-800-633-8100 to get a referral to a
physician or a specialist. Specialist
referrals are made after a patient has seen a primary care physician who
provides a referral to a specialist.
How To Get Medications
Participating physicians write prescriptions from the list of available
medications. You can take the prescription and your eligibility form to one of
the 500 participating pharmacies across the state and have the prescription
filled for free. To find a participating pharmacy in your area, call the hot
line at 1-800-633-8100. As long as you are eligible for the program, you can
access as many medications as a physician believes is necessary.

This organization offers Supported Living Services with the goal of helping individuals remain in the community, avoiding homelessness and long-term hospitalization. Supported Living Services is a voluntary program where assistance and specific needs are identified and and then assistance is provided to address those identified needs. Individuals are given assistance in locating suitable housing, applying for Medicaid, applying for food stamps and getting medical attention which could include psychiatric care.
NorthKey Community Care
NorthKey Community Care provides assistance to individuals suffering from
emotional problems or mental illness. Services include psychological,
psychiatric and medication evaluation and treatment.
